1. Certificate Replacement Request Form

  • The information must be complete, with no cross-outs or alterations.
  • The form is available in an editable PDF format. Once completed, you must print it, sign it, and submit it along with the required documents.
  • You can also complete the process using a national digital signature that has been properly certified by the appropriate institution; the digital certificate allows its owner to authenticate themselves and sign electronically. It is not a scanned signature.

2. Photocopy of valid identity card

  • The photocopy must be legible.

3. Cancellation of the Corresponding Right

  • The cancellation fee for this right is ₡20,000 colones.
