1. The applicant must
- Be licensed to practice.
- Be up to date with tuition fees for the month in which the application is submitted.
- The information must be complete, with no cross-outs or alterations.
- You can also complete the process using a national digital signature that has been properly certified by the appropriate institution; the digital certificate allows its owner to authenticate themselves and sign electronically. It is not a scanned signature.
- The form is available in an editable PDF format. Once completed, you must print it, sign it, and submit it along with the required documents.

4. Photocopy of valid identity card
- La fotocopia debe ser legible y por ambos lados.
5. A legible photocopy of the ticket you will use for your trip or any other document proving your stay outside the country and the length of your absence, supporting the information provided on the form.
- En caso de ser requerido, el CFIA solicitará la documentación en idioma español, por lo que se deberá aportar traducción oficial debidamente autenticada.
Please note that:
- All information and requirements must be submitted in full. If any of the established requirements are not provided correctly or do not comply with CFIA regulations, the application will not be processed and will have no legal effect.
