1. The applicant must
- Be licensed to practice.
- Be up to date with tuition fees for the month in which the application is submitted.
- The information must be complete, with no cross-outs or alterations.
- The procedure can also be carried out with a national digital signature duly accredited by the corresponding institution. The digital certificate allows its owner to authenticate themselves and sign electronically. It is not a scanned signature.
- The form is available in an editable PDF format. Once completed, you must print it, sign it, and submit it along with the required documents.

4. Photocopy of valid identity card
- The photocopy must be legible and include both sides.
5. A legible photocopy of the ticket you will use for your trip or any other document proving your stay outside the country and the length of your absence, supporting the information provided on the form.
- If required, the CFIA will request documentation in Spanish, so an officially certified translation must be provided.
Please note that:
- All required information and documentation must be submitted in full. If any of the specified requirements are not met correctly or do not comply with CFIA regulations, the application will not be processed and will have no legal effect.
