The application can be submitted by email to dep.registro@cfia.cr or in person at the CFIA Service Platforms.

The applicant must:

  • Be licensed to practice.
  • Stay up to date with your obligations to the CFIA.

1. Certificate Replacement Request Form

  • The information must be complete, with no cross-outs or alterations.
  • The form is available in an editable PDF format. Once completed, you must print it, sign it, and submit it along with the required documents.
  • The procedure can also be carried out with a national digital signature duly accredited by the corresponding institution. The digital certificate allows its owner to authenticate themselves and sign electronically. It is not a scanned signature.

2. Photocopy of valid identity card

  • The photocopy must be legible.

3. Cancellation of the Corresponding Right

  • The cancellation fee for this right is ₡20,000 colones.

The process takes three business days, provided that all required documents have been submitted in accordance with the established requirements.

The replacement certificate of incorporation will be delivered, without exception, in person to the applicant, who must present their original valid identity card.

Please note that:

  • All required information and documentation must be submitted in full. If any of the specified requirements are not met correctly or do not comply with CFIA regulations, the application will not be processed and will have no legal effect.
